What is Endoscopic Sinus Surgery?

Endoscopic Sinus Surgery (ESS) is a minimally invasive procedure that clears blocked sinus drainage pathways to treat chronic sinusitis and nasal polyps that have not responded to medication. It is performed entirely through the nostrils using a small camera (endoscope), with no external cuts or facial scars.

The sinuses are air-filled spaces around the nose that drain through narrow channels. In chronic sinusitis, these channels become blocked and inflamed, trapping mucus and causing repeated infection, congestion, facial pressure, and reduced sense of smell. ESS uses a thin endoscope passed through the nostril to open and widen these drainage pathways and, when present, to remove nasal polyps. Surgery is considered when medical treatment has not given enough relief.

Signs you may need sinus surgery

ESS may be considered when chronic sinus symptoms persist despite proper medical treatment, such as:

  • ✓Repeated or long-lasting sinus infections (chronic rhinosinusitis)
  • ✓Constant nasal blockage or congestion
  • ✓Facial pain or pressure around the cheeks, eyes, or forehead
  • ✓Nasal polyps confirmed on examination or CT scan
  • ✓Thick nasal or post-nasal discharge that does not improve with medication

Risks & What to Discuss with Your Surgeon

ESS is generally safe and is performed close to the eyes and the base of the skull, which is why it is done with an endoscope for precise visualization. Risks are uncommon and include bleeding, infection, or swelling. Following the saline-rinse and follow-up plan is an important part of a good result. Your surgeon will review the risks specific to your sinuses and CT scan findings before surgery.
