1. Appointment Policy

All consultations are by appointment only. Please contact us via Facebook or by calling our clinic to schedule your visit.

• Appointments are subject to availability
• Confirmation of appointment is required
• Please arrive 10-15 minutes before your scheduled appointment
• Bring valid government-issued ID

2. Cancellation & Rescheduling

We understand that schedules change. Please observe the following cancellation policy:

• 24-hour notice: Free cancellation or rescheduling if requested at least 24 hours before appointment
• Less than 24 hours: A cancellation fee may apply
• No-show: Failure to appear without notification may incur charges
• Rescheduling requests are subject to availability

3. Payment & Fees

Payment for services rendered is expected at the time of visit unless other arrangements have been made.

• We accept cash and bank transfer only
• We do not accept HMOs, GCash, or credit/debit cards
• Late or unpaid accounts may be referred to collection
• Itemized receipts will be provided for all transactions

4. Medical Disclaimer

Dr. Jennifer Almelor-Alzaga is a licensed Otolaryngologist (ENT Specialist) and Head & Neck Surgeon practicing in accordance with Philippine medical standards and regulations.

• All medical services are provided as professional medical opinion
• Diagnosis and treatment are based on clinical assessment and examination
• Results and outcomes cannot be guaranteed and vary by individual
• Patients should seek a second opinion if uncertain about any treatment recommendations
• Emergency medical conditions require immediate hospital care, not our clinic

5. Patient Responsibilities

To ensure the best care possible, patients are responsible for:

• Providing accurate medical history and information
• Disclosing all current medications and supplements
• Informing us of any allergies or adverse reactions
• Following post-operative or post-treatment instructions carefully
• Reporting any complications or concerns immediately
• Maintaining good communication with clinic staff

6. Medical Records & Privacy

We are committed to protecting your privacy and maintaining the confidentiality of your medical records in accordance with Philippine Data Privacy Act and medical ethics standards.

• Medical records are kept confidential and secure
• Records may be shared with authorized healthcare providers as needed for treatment
• Patient consent is required for release of records to third parties
• You have the right to access your medical records
• We do not sell or share personal information without consent

7. Limitation of Liability

To the extent permitted by law, Dr. Jennifer Almelor-Alzaga and ENT Manila staff shall not be liable for:

• Indirect, incidental, or consequential damages arising from treatment
• Outcomes that fall outside the standard of care for the procedure
• Complications arising from patient non-compliance with instructions
• Third-party services or referrals

Medical malpractice claims must be pursued through proper legal channels in accordance with Philippine law.

8. Consent to Treatment

By scheduling an appointment and proceeding with examination or treatment, you acknowledge that:

• You have read and understood these terms
• You consent to physical examination and diagnostic procedures
• You have disclosed all relevant medical information
• You understand the risks and benefits of recommended treatment
• You agree to follow medical advice and post-care instructions

9. Referrals & Second Opinions

Patients have the right to seek a second opinion or be referred to another specialist. We support informed decision-making and welcome patient autonomy in their healthcare choices.

10. Modifications to Terms

We reserve the right to update these terms at any time. Continued use of our services constitutes acceptance of updated terms.
